Family Home Health is a growing home health agency owned and operated in Boulder, Colorado. We are growing and have a need for a Community Liaison to further develop our northern territory which includes Fort Collins, Greeley, Loveland, Longmont and the surrounding areas.

Responsibilities for the Community Liaison role include:

  • Coordinating referrals for home care services
  • Establishing new contacts with referral sources within the community
  • Strengthening relationships with past referral sources
  • Provides updates on referrals at weekly marketing meetings

The ideal candidate in this role will needs to be self-directed and able to work with minimal supervision. Prior marketing/community outreach/sales experience in the healthcare industry is required with prior home health experience preferred. Strong communications, presentation and interpersonal skills are essential.

This is a full-time position with health/dental/vision/retirement benefits, paid time off, mileage reimbursement and monthly bonus based on referrals.

Note: even though the work is in the northern Colorado area, interviews with potential candidates would take place in our main office in Boulder so please take this information into consideration when applying.

*$1000 bonus – $500 will be paid after three months of continuous employment and $500 will be paid after six months of continuous employment.
